What it is

Clinical assessment that funders can act on

Assessments and reports are a distinct service line at Constellation, not an afterthought. Our senior clinicians do a lot of this work, so we know what NDIA delegates, plan managers, aged care assessors and insurers are looking for: clear functional evidence, recommendations mapped to plan categories, and language that holds up to review.

We do assessment-only work where you don't need ongoing therapy with us. That's a deliberate choice. Sometimes a family or support coordinator just needs a clean report from an independent clinician so the plan review or access request can move forward.

Most assessments include time with the participant or client, time with family or support staff where relevant, file review, scoring (where applicable) and a written report. We agree the scope, timeframe and fee in writing before we start.

Assessments are fundable through NDIS, Aged Care, DVA, insurance and private payment. We quote in writing before we start.

NDIS

Plan-managed, self-managed and agency-managed. Capacity Building, Improved Daily Living and Core supports.

Aged Care & Support at Home

Allied health under Support at Home, home care packages and aged care home placements. In-home and on-site visits.

DVA

Veteran allied health under valid DVA referrals. Bulk-billed where eligible.

Private

Pay privately for therapy or assessments without a referral. Clear, upfront fee schedule.

Common questions

Assessments & Reports FAQs
Do I need a GP referral for an FCA?

No. NDIS participants, support coordinators and plan managers can book a Functional Capacity Assessment directly through our intake. We do ask for background information and any current reports so we don't duplicate work that's already been paid for.

How long is a Functional Capacity Assessment?

An FCA usually involves a one-to-two hour appointment (often in the home), follow-up where needed, and a written report. We agree the turnaround in writing. Most FCA reports are with you within two to four weeks of the assessment.

What's the difference between an FCA and an NDIS Clinical Psychology Assessment?

An FCA is OT-led and focuses on functional capacity across daily life. An NDIS Clinical Psychology Assessment is psychologist-led and focuses on psychological diagnosis and psychosocial disability evidence. They answer different questions. We'll help you work out which one (or both) your situation calls for.

Can I get an assessment without committing to ongoing therapy?

Yes. A lot of our assessment work is assessment-only. Sometimes the most useful thing we can do for a family is provide a clean, independent report and hand them back to their existing team. We'll never pressure ongoing therapy as a condition of doing an assessment.

How much do NDIS assessments cost?

Assessments are quoted in writing before we start, based on scope, complexity and discipline. NDIS assessments are billed at NDIS Pricing Arrangements rates. Most FCAs and Clinical Psychology Assessments sit within standard ranges that support coordinators and plan managers will recognise.

Can you do assessments interstate?

Yes. Our mobile clinicians cover greater Melbourne, Sydney, Perth, Adelaide, Darwin and SE Queensland for face-to-face assessment, and we use telehealth for parts of the process where it's clinically appropriate. We're upfront at intake about what's possible in your location.

Will the report use language the NDIA will actually accept?

Yes. Our senior clinicians write a lot of this work and we know how the NDIA reads reports. We focus on functional evidence, plain language and recommendations mapped to NDIS plan categories. Support coordinators are welcome to call us before booking to talk through the specific decision the report needs to support.
